Starting January 1, 2027, millions of Medicaid beneficiaries will be subject to new requirements under HR 1, also known as the “One Big Beautiful Bill Act,” signed into law by Donald Trump on July 4, 2025. The law mandates that individuals in the Medicaid expansion population in 44 states must prove they are working, attending school, or volunteering at least 80 hours per month, or earning $580 monthly. Eligibility will also need to be re-certified every six months, instead of annually, making coverage harder to obtain and maintain. According to the Kaiser Family Foundation, over 82 million Americans rely on Medicaid. Advocates and healthcare providers express concern that these changes will lead to increased medical debt, delayed care, and more individuals losing coverage. Dr. Andrew Racine, president of the American Academy of Pediatrics, stated, “The costs to the family – and society as a whole – cannot be overstated.” The law also reduces retroactive Medicaid coverage from three months to two, or one, depending on the applicant and state. The Urban Institute estimates HR 1 will result in $278 billion in uncompensated care by 2034, potentially straining hospitals, particularly in rural areas.

Starting in 2027, if you're on Medicaid, you won't just have to be poor; you'll have to prove you're also working, going to school, or volunteering for 80 hours a month, or earning a princely 580 bucks. That's the kind of beautiful, big-league thinking you get from a man who thinks a hospital is a place you go to sign a confidentiality agreement after catching a venereal disease from a porn star.
