The facts
On October 7, 2026, the FDA issued warning letter CMS #729208 to Olympus Medical Systems Corporation after inspecting six of its facilities: Hachioji, Aomori, Hinode, Shirakawa, Hamburg, and Brooklyn Park. The company makes hemostatic metal clips, sphincterotomes, cystoscopes, bronchoscopes, endoscopic distal covers, valves, and insufflators. The FDA found the devices "adulterated", citing "the distribution of nonconforming devices and significant risks to patients."
The UHI-4 insufflator is the centerpiece. Olympus "failed to identify and control known catastrophic hazards", which the letter names as "air embolism, arrhythmia, and patient death," and those hazards were identified through "Health Hazard Assessments and complaint investigations". This was even "after a confirmed patient death and Class I recall of UHI-4 units in 2023." UHI-4 complaints included "cardiac arrest and possible gas embolism", which were "not fully evaluated". One complaint "reported a death due to gas embolism during use and included the device event log", which "was not reviewed as part of the investigation."
The BF-1TH190 bronchoscope drew a class I recall over "a risk of endobronchial combustion if high-frequency cauterization is performed while supplying oxygen". If it happens, patients "may suffer adverse events, including internal burns in their airway or lungs." Olympus had claimed laser compatibility in the instructions "without conducting design validation testing".
The Vizishot 2 FLEX (19G) needle detached inside patients, including one case "where the device may have contributed to a patient death during explant surgery to recover the detached needle." Olympus reworked and redistributed the devices "without adequate justification", and "circumvented the ship hold in place under the NCR by changing the manufacturing date during rework,"
The Thunderbeat 5 mm device drew "over 1900 complaints between September 2021 and September 2024, including 1583 that were MDR reportable." Olympus opened a corrective action in October 2023, but "no action was taken as the Health Hazard Evaluation was canceled on January 31, 2024, because it was deemed not necessary based on complaint incident rates and without considering the potential harm to patients." The recall came "on November 25, 2024, over a year later."
On the factory floor, inspectors found failing parts retested until they passed: 8 of 10 samples were retested, and the only rationale recorded was "Believed fixture incorrect." Failing parts were "accepted into inventory." One lot "was accepted by mistake".
On reporting, a resection set with "broken ceramic insulation" malfunctioned in use, and a 2019 report "documents a serious injury resulting from the same malfunction involving the same device." Two later complaints were reported late: both "were submitted beyond the required 30-calendar-day reporting timeframe." "This is a repeat violation from our 2023 Warning Letter issued to the Hinode facility." and the letter notes "three prior warning letters" issued to the firm in 2022 and 2023. The FDA warns that unaddressed violations "may result in the FDA refusing admission of the devices manufactured by your firm into the United States", and that adulterated devices "may be detained or refused admission."
Our take
Let’s get this straight: the company that makes the fucking tools doctors shove into your lungs and guts has a documented quality control system that amounts to a shrug and a note that says 'Believed fixture incorrect.' That’s it. That’s their excuse for why eight out of ten failing parts got retested until they passed and then were accepted into inventory. One lot was just accepted 'by mistake.' This isn't a medical device manufacturer; it's a clown show with a shipping department, and the clowns are packing gas embolisms and airway fires into sterile packaging.
They have a fucking insufflator, the UHI-4, that they knew could cause air embolisms, arrhythmia, and patient death. They knew this after a confirmed patient death and a Class I recall in 2023. And what did these absolute cunts do? They got a complaint about another death with a device event log attached, and they didn't even review the fucking log as part of the investigation. A man is dead, his body logged the murder weapon's confession, and Olympus couldn't be arsed to hit 'play.' That’s not negligence; that’s pathological, profit-driven sociopathy.
Then there’s the bronchoscope. They printed in the instructions that it was laser compatible. They did zero design validation testing to see if that was true. The result? A 'risk of endobronchial combustion.' That’s FDA-speak for 'your patient's lungs might catch fire if you use oxygen near the cauterizing tool.' They sold a device that could literally light someone's insides on fire based on a marketing hunch. That’s not an engineering failure; it’s attempted murder with a spec sheet.
Their Thunderbeat device racked up over 1,900 complaints, 1,583 of them legally required to be reported. They opened a 'corrective action' in October 2023 and then fucking canceled the health hazard evaluation two months later because the complaint rate wasn't high enough for them. The letter explicitly states they did this 'without considering the potential harm to patients.' The potential harm to patients was not a variable in their equation. They are playing spreadsheet roulette with people's lives, and the 'house edge' is a pile of corpses. The recall finally happened over a year later, a leisurely pace for a company that treats human suffering as a quarterly accounting problem.
This is their fourth warning letter. The previous three, in 2022 and 2023, were apparently just polite suggestions they wiped their asses with. They circumvented shipping holds by changing manufacturing dates on reworked needles that were detaching inside people - one possibly contributing to a death during the surgery to retrieve it. They filed injury reports late, a 'repeat violation.' The FDA is now threatening to block their devices at the border, which is the regulatory equivalent of finally telling the arsonist he can't bring his own gasoline into the house he's already burning down. The only thing more terrifying than an Olympus device inside you is the documented thought process of the prick who approved it for shipment.
